Forms

Notice of Intended Marriage Form
The Notice of Intended Marriage (NOIM) needs to be completed prior to the marriage. Your Celebrant MUST receive this completed form at least one month and one day before the intended date of your wedding. An authorised person, one of whom can be your marriage celebrant, must witness your signatures.

If meeting in person I will supply you the Notice of Intended Marriage form at our first meeting where we will complete it together. Or you can click on the link to download the NOIM yourself. If you were born in Australia you must produce your original birth certificate or official extract - photocopies are not acceptable.

If you were born outside of Australia you are required to produce a birth certificate. However, if you do not have a birth certificate I can accept your overseas Passport. If you were previously married you must produce the Divorce Decree Absolute or Death Certificate, whichever is applicable.

Prior to your wedding both partners will be required to sign a declaration, stating that you believe there is no legal impediment to the marriage between yourself and your partner.

Standard Marriage Certificate
To change your surname on your driving licence, passport and some other legal documents, you will need another kind of marriage certificate called the Standard Marriage Certificate. This is a different certificate to the one given to you on your wedding day. This can only be obtained from Births, Deaths & Marriages after your wedding and this certificate shows the registration number of your wedding. Visit http://www.justice.qld.gov.au/16.htm to apply for Standard Marriage Certificate. Go to certificates and download forms.
